This application with the required documentation must be submitted to D&SDT-Headmaster at least one (1) full business day before your scheduled test date.
NAME CHANGE - UPDATE OR CORRECTION TO SPELLING | CORRECTION TO DATE OF BIRTH
You must upload one of the following documents for a name change or to correct the spelling of your name or date of birth:
- An image of your valid US government-issued photo identification, such as:
- State Driver’s License,
- Passport,
- or other signed, current, photo identification
For a name change, please also upload one of the following proof of legal name change documents:
- Marriage License
- Divorce Decree
- Court Documents of Name Change
